REGISTRATION INFORMATION FOR
FALL 2009/SPRING 2010 SEASONS!!
Registration for the Fall 2009/Spring 2010 seasons is now open. For your child to be officially registered, you must attend one of the following registration sessions with all the following requirements. Registration sessions will be held on the following dates only:
Tuesday, March 24, from 6:30pm until 8:30pm
Saturday, March 28, from 1:00pm until 4:00pm
Saturday, April 4, from 10:00am until 1:00pm
Saturday, May 2nd, from 4:00pm until 6:00pm
Saturday, May 30th, from 10:00am until 12:00pm at Halmich Park
Tuesday, June 23, from 6:30pm until 7:30pm
Thursday, June 25, from 6:30pm until 7:30pm
Registration sessions will be held at the Warren Community Center only on the date and times stated above. Please do not call the Warren Community Center as they only offer space to AYSO and are not affiliated with us. The Warren Community Center’s address is 5460 Arden Road, which is located South of 14 Mile Road, on the West side of Mound.
**NO PLAYER IS OFFICIALLY REGISTERED UNTIL YOU ATTEND A REGISTRATION SESSION WITH THE FOLLOWING REQUIREMENTS**
WE ARE RECOMMENDING PRE-REGISTRATION FOR ALL NEW PLAYERS AND ALL NEW VOLUNTEERS ONLY. Even though you pre-register, your child is not officially registered until you bring 3 copies of your pre-registration form and the above requirements to register your child, to one of the registration sessions. If you are volunteering, you will only need to take 2 copies of your volunteer form. If you want copies of your pre-registration/volunteer forms for yourself, you will need to print additional copies. Click here to pre-register at eayso.org. We will have pre-printed registration forms for current season players and volunteers.
ALL VOLUNTEERS ARE REQUIRED TO FILL OUT A VOLUNTEER FORM-That includes coaches, assistant coaches, referees, team parents, and board members. In addition each family will be required to fill out a volunteer survey form.
REQUIREMENTS TO REGISTER NEW AND RETURNING PLAYERS – The following information is necessary for all players:
1. Your Child’s Birth Certificate – used to verify your child’s Date of Birth and will be returned at the session.
2. A Current 1 ½“ X 2” Photo of Your Child – This will be used for the player ID card.
3. Emergency Contact Person’s Name and Phone Number – This must be someoneother than you or living in your household.
4. Physician’s Name and Phone Number.
5. Medical Insurance Carrier – Necessary in case of emergency. If you do not have medical insurance, please write NONE in the space provided on the registration form.
6. Cash, Check, Money Order, Visa, or Mastercard – All will be accepted at any registration session. Please make payment out to AYSO Region 708. There will be a $25 charge for all returned checks. Online payments will not be accepted.
REGISTRATION FEE FOR U06 TO U14 DIVISIONS:
$85.00 per player along with 2 books of 20 raffle tickets. Sell the raffle tickets, keep that money, return the ticket stubs, and your player fee will end up only costing $5.00 if you sell all tickets!!! The number of raffle tickets sold determines what you receive back.
**No additional player/family discount will be offered.
**These fees are for two seasons (20 Games).
REGISTRATION FEE FOR U16 AND U19 DIVISIONS:
$45.00 per player along with 1 book of 20 raffle tickets, per season. Sell the raffle tickets, keep that money, return the ticket stubs, and your player fee will end up only costing $5.00 per season if you sell all tickets!!! The number of raffle tickets sold determines what you receive back.
**No additional player/family discount will be offered.
**These fees are for each season (10 Games). To play the second season (10 Games), players must pay same fees and raffle tickets upon returning for second season.
Additional Raffle Information
Drawing is on March 27th, 2010 at Noon
First prize is $200.00
2 & 3 are $50.00
4-11 is for $25.00.
Sell the raffle tickets and return the stubs.
Return ticket stubs to coaches and/or Board Members. The turn-in date will by Feb 2010.

BE A HERO IN A CHILD’S LIFE - VOLUNTEER! AYSO Region 708 is an all volunteer organization. If you would like to volunteer some of your time to our region, please send an e-mail to the Commissioner at commissioner@ayso708.org or you can inquire about volunteer descriptions at the volunteer table at one of the registration sessions. We would not be able to have a region without the help of many volunteers. Please take the time to consider volunteering your services to help provide a well rounded program for our kids. It is rewarding working with the kids as they improve each season, and AYSO provides all necessary training. Many positions are available as coaches, referees, board members, and other appreciated positions. Help us give as many children as possible the opportunity to play this wonderful sport by volunteering some of your time.
